Well, that's the AI for the Gobbler phase of the fight... the Kefka phase uses different AI that shouldn't have Bolt 3 or Ice 3. In any case, you were able to survive Goner pretty well, so I don't think it would have changed the fight much.
Regarding the missing super boss... that is strange. I was pretty sure that all super bosses end up as the rare encounter in at least one pack. They can also end up in chests, but not always. The way Beyond Chaos is now, it is possible to miss some other monsters if they're not super bosses. In the future I'm going to try to improve monster formation coverage, since I can now easily determine the locations of monsters.
Actually... this was really bothering me, so I decided to look it up. It seems like the monster you're looking for should've been a random encounter in Kefka's Tower, in the tiny 1-screen area that looks like the Phantom Forest. The zone is 183.
