FFIV Boss battle music patch and FFIV Battle Music patch should only affect the C5 bank where the instruments numbers used for each song are. As for the song data it should have changed bytes in bank C8 and C9 depending which songs they are replacing. As for Poco's patch, it's a graphic patch and shouldn't enter in conflict with any of the other two. If the 3 patches have been proved to work correctly, it could be the fact that your ROM has a header and one or all 3 patches are made for a header-less ROM, or vice versa. This would have as effect to change other data than what it's suppose to.

If you enter the first battle correctly, maybe something related to the battle formation or mould of the second formation has been corrupted. You can always run a report with Lunar IPS. You simply take a clean ROM and by having the patch in the same folder, you apply the patch. A text file with the bytes changed will appear. Repeat this process with 2 other copies of your clean ROM and the 2 other patches and you'll be able to see if there is a conflict or which data has been changed.
