I have to say the lack of a "world map" is a big turn off. Even if tuere is no real reason to have it, and only one place to go to advance the story, I still want the illusion of freedom.

Little bastard NPC that insist on walking in your way, and always get trapped in the one place where there is no way to go around them or push them forward.... hallways, doors, etc. But that's why fast forward rocks.

My biggest turn off is lack of character connection. It doesnt have to be just one main character, but the games that jerk you around to so many different main characters that you dont really give a damn about any of them... that is my biggest rage, hit the power button with a hammer and burn the game disk to ashes thing. I have to give a damn about the characters to care any about the story.
